What is the root cause of the error cannot insert update activate entity salesforce?

A Veeva trigger wants to locate the Account information within the importing file plus utilize this Account for creating a Survey Target record. If that survey target Account ID seems not to exist on Online CRM, that error message displays cannot insert update activate entity.
